About Us
Phare Care Services offers domiciliary tailored approach care supporting a wide range of client groups ranging from people diagnosed with dementia, mental health needs, learning and/or physical disabilities and complex medical needs. This is done in a way that ensures the social, physical and emotional needs of our clients are met.
We provide our clients with a team of care workers, nurses and support staff, who go above and beyond in providing outstanding care for our clients. We believe in providing an environment that encourages and supports clients to lead as full and active a life as they are able, within their environment.
Ultimately our aim to keep clients in their own homes for as long as possible. Home is where people feel comfortable, safe and secure and where they want to be, instead of having to go into residential care.
Our Philosophy
Care
Phare Care Services provides a full range of home care services, including specialist care for complex conditions. We aim to tailor the care we provide to our clients’ needs ensuring their care is personal to them. Our staff are experienced in managing and addressing both health and social care needs of all ranges of clients.
Integrate
At Phare Care Services we believe in integrated care which aims to provide person-centred support enabling different health care teams to work together more closely and for the individual to receive care that caters for all of their needs. This is done by working closely in partnership with individuals, their families or people important to them to ensure we develop and deliver a package of support which is unique to each person we support and care for.
